Annual Re-registration for Returning Students
Current Crestview families can complete annual back-to-school and athletics forms online using FinalForms. This link can also be used for mid-year updates to student information.
Account Creation
Student signatures: Grades 7-12 students may use their district-assigned email address to sign into FinalForms to complete the student signature portion of the forms. Students should use the "Never logged in" or "Forgot password" options to obtain a link to create a new password. A district-assigned password is not set for students.
Missing Parent Email Addresses: A parent within each household should have received a FinalForms invitation. Families who did not receive an invitation may not have an email address on file. Athletic Forms
Our Crestview Athletics Packet is now incorporated within FinalForms online forms. In addition to being able to submit all of the required OHSAA documents, student health information will carry over onto the printable OHSAA Physical questionnaire packet that families can take to the student's sports physical. In future years, families will need only to update the previous year's content to complete the requirements.

New Enrollment
Thank you for your interest in enrolling at Crestview Local Schools. We are proud to serve preschool through high school students in Richland and Ashland counties. We look forward to getting to know your child and your family.
To begin the enrollment process, for either K-12 or preschool, please review the steps below. If you are inquiring about Open Enrollment, scroll down for more information.
- Complete Registration Online
Crestview uses an online registration process to gather the needed information to enroll your child in school. To begin enrollment, follow the process below.
- Click on the link for Final Forms: https://crestviewashland-oh.finalforms.com/.
- Click on the Parent (New Account) button on the Final Forms site. You will be instructed to create a login ID and password. You can save your information and return at a later time for completion if needed.
- Complete all forms and upload the required documents (you can also Fax or email these documents if preferred).
- Required Documents
- Student Birth certificate (color copy of the original certified birth certificate)
- Student Immunization record (Link: Immunization Requirements)
- Photo ID of parent/guardian
- Two (2) proofs of residency from the following list
- Utility bill (electric or gas)
- Tax bill
- Purchase agreement for new home
- Lease agreement for rental property
- Rental or homeowner’s insurance policy or statement
- Paycheck/employer letter (on employer letterhead)
- Voter registration card
- Driver’s license with current Crestview district address
- Residency affidavit
- Foster placed students will also require the journal entry from the court
- Court papers (legal custody or change of custody) if applicable - **All custody paperwork must but be court-stamped by a family court magistrate or judge. Documents without an official court stamp will not be accepted.**
- Enrollment Completion
- Once your Registration is complete and all needed documentation received, you will be contacted by the building for any needed information for scheduling and to inform you of your students start date.

Crestview Preschool Registration
Crestview is pleased to offer an on-site preschool program housed at Crestview Elementary School and operated in conjunction with Warren County Educational Service Center.
The purpose of the preschool is to provide an enriched learning environment in an integrated setting to children, ages 3-5, by August 1.
For parents who are interested in enrolling your child in Crestview’s preschool, please call 419-895-1700, extension 17002 to inquire about the registration process. All registrations should be completed prior to preschool screening. Community preschool screenings will be held on April 1, 2022. Information required for registration is: Birth Certificate, current Immunization Record, Custody Papers (if applicable), and Two Proofs of Residency in the Crestview School District. For a complete list of acceptable proofs of residency, visit our website at www.crestviewschools.net, click on “Registration” link. Once the required documents have been presented, you and your child(ren) will be set up in Final Forms, our online enrollment system. You may complete these forms from home or in our office if you don’t have access to a computer at home.

Open Enrollment
Students living outside the Crestview School District can apply to attend Crestview as an open enrollment student.
Parents who wish to send their student to Crestview under open enrollment must complete the necessary application process and obtain board approval before being enrolled. Please contact the Board of Education office for more details. Further information on the process is included in the packet linked below.

Open Enrollments for 2022-2023 School Year
It's that time of year again to fill out open enrollment forms. Families who open enroll their children into the district must fill out a new form every year if they wish for their child to stay in the district.
The Open Enrollment Application process for the 2022-2023 school year begins April 1 and runs through May 1, 2022.
All applications will be reviewed and accepted at the discretion of the Superintendent. All applicants, accepted or denied, will be notified by mail on or before June 20, 2022. Accepted applicants NEW to Crestview LSD will be instructed to schedule an appointment in our registration office located in the central office. Anyone having questions about the open enrollment process can contact the Superintendent's Office at 419-895-1700, ext. 17000.

If you live in a non-adjacent school district to Crestview LSD and are not eligible for Open Enrollment, a parent/guardian can apply to pay tuition for your child to attend Crestview LSD.
