Crestview Local Schools Return to Play Protocol following a Covid Positive Test for Athlete's

1.  14 Day Quarantine 

2.  Athlete must receive clearance from a Physician to return to athletic activity - (Use OHSAA Covid Clearance Form)

3.  Upon Physician clearance the athlete will then enter a 6 day Return to play protocol with our AVITA Athletic Trainer

4.  Once our Athletic Trainer has completed the return to play protocol with our student athlete the athlete will be reinstated into competition.
