When should my child stay home?

Crestview Local Schools want every child to learn and be successful. School attendance is linked to learning. However, there are times when your child may be ill and should stay at home until he/she feels better or is no longer contagious to others. Below are some guidelines to help you determine when your child should stay home.

 

  • Temperature above 100° by mouth
  • Vomiting or diarrhea
  • Shortness of breath or wheezing during normal activity
  • Cough that interrupts normal activity
  • Pain from earache, sore throat, or recent injury
  • Yellow or green drainage from eye(s)
  • Rash or draining sores

Screening Information

HEALTH ASSESSMENT, SCREENING, AND REFERRAL INFORMATION
The health screening is the school nurse's way of checking students for possible health problems that might interfere with the child's classroom performance. Screenings may include the child's health history, parent and teacher observations and the nurse's professional evaluation. The school nurse will make a referral if any concerns are identified.

When and what health screenings will be done on my child at School?
Ohio law 3313.69 requires school nurses to conduct periodic health screenings. Screenings will be conducted on all new students to the district and at the following grade levels:

  • If you child needs vaccines to meet the State of Ohio school vaccination requirements:
    • Visit your family physician or clinic
    • Contact the Public Health Department

Guidelines for Medications

If medication needs be taken during the school day please follow these guidelines:
Have a completed, signed Medication Authorization Form.
The parent/guardian must complete the bottom section and a healthcare provider must complete the medication order section.
A new Medication Authorization Form must be completed each school year or when there is a change in the medication or dose.
All medication must be in the original container in which it was dispensed by the prescribing physician, healthcare provider or licensed pharmacist.

    • The container must be labeled with the correct dose and instructions. 
    • The label should match what is on the Medication Authorization Form. 
    • Students taking medication at both school and home can request 2 separate labeled bottles from the pharmacy to divide the pills to have some at home and school.

School personnel cannot give students over-the-counter medications unless prescribed by a healthcare provider and a Medication Authorization Form is completed.
Prescribed over the counter medications follow the same guidelines as stated above for prescribed medications. (Over the counter medications include pain medication such as Tylenol, cough medicine, ointments.)
Medications ordered three times a day or less, unless there is a time specified, may not need to be given at school.
The medication should be given before school, after school and at bedtime.

All left over medication must be picked up by the parent or guardian on the last day of school or it will be disposed of properly.
